Pen Injector
A pen injector is a pen-shaped device holding a drug cartridge and a dose-setting mechanism that advances a plunger by a fixed increment, delivering a measured volume through a fine needle.
A pen injector combines a sealed cartridge, a mechanical dose-setting mechanism and a fine needle in one hand-held device. Turning the dial engages a lead screw that will advance the plunger by a fixed linear distance when the button is pressed; because the cartridge bore is constant, that distance corresponds to a fixed volume, and the volume corresponds to a labelled amount only at the concentration in that cartridge. Doses are therefore quantised: a pen delivers its increments and nothing between them.
The format dates to the mid-1980s, when the first insulin pen made multiple daily injections practical outside hospital, and it now carries most self-administered peptides. Fixed-dial pens are used for the GLP-1 receptor agonists, where the label defines which settings exist rather than offering a continuous scale. Pen needles are short and fine, in the four to eight millimetre range at gauges around thirty, which works only because the device controls depth. Because the cartridge is entered repeatedly, pen formulations are preserved.
The tradeoff is precision against granularity. A pen delivers small volumes more accurately than a hand-drawn syringe and removes several error-prone steps, which is why device-related errors fall when a product moves to a pen. What it gives up is any amount the mechanism does not offer, and it ties the drug to one concentration for its shelf life.
The recurring error is treating a dial click as a universal unit. An increment means an amount only for that product at that concentration, so numbers quoted between different pens, or copied onto a reconstituted vial, are meaningless. A second error, common in grey-market advice, is withdrawing cartridge contents into a separate syringe: the cartridge was never dose-verified for that use, and its preservative system assumes an intact closure.
