Benzyl Alcohol Preservative

Benzyl alcohol is an aromatic alcohol used at around 0.9 percent as the antimicrobial preservative in multi-dose injectable containers, where it also acts as a mild local anaesthetic.

Benzyl alcohol is one of a small set of preservatives compendially acceptable in parenteral products, alongside phenol, metacresol and the parabens. It disrupts microbial membranes and is bacteriostatic rather than bactericidal, holding a low-level contaminant in check rather than sterilising a container. Its secondary property is noticeable in use: it is a weak local anaesthetic, so a preserved formulation often stings less than the same drug in a preservative-free vehicle.

Regulators expect any multi-dose container to demonstrate preservative efficacy under a compendial antimicrobial effectiveness test using defined challenge organisms. That requirement is why multi-dose vials are preserved and single-dose vials generally are not. The best-known harm signal is neonatal gasping syndrome in the early 1980s, in which preterm infants exposed to benzyl-alcohol-containing flush solutions developed metabolic acidosis, respiratory depression and death.

For a peptide the choice is a real tradeoff. A preservative buys repeated entry into a container without the microbial risk governing preservative-free presentations, but benzyl alcohol is a small amphiphile that partitions into hydrophobic protein surfaces and has been shown to promote aggregation and particle formation in several therapeutic proteins. Development therefore treats the preservative as an active variable in stability testing.

The mistake seen most often is inferring that a preserved vehicle makes a preparation indefinitely usable. Preservative efficacy is defined against a specified challenge under specified storage, weakens with dilution, and says nothing about chemical degradation or endotoxin. Hypersensitivity to benzyl alcohol itself is documented, and is routinely overlooked when a reaction after injection is attributed automatically to the active ingredient.
