Bacteriostatic Water vs Sterile Water
Bacteriostatic water is sterile water containing an antimicrobial preservative that allows a container to be entered more than once, while sterile water for injection is preservative-free and single-entry.
Both are pharmacopoeial articles of water for injection, and the only difference between them is a preservative. Sterile Water for Injection contains nothing else at all: no buffer, no tonicity agent, no antimicrobial. Bacteriostatic Water for Injection is the same water with benzyl alcohol added at 0.9 percent, which inhibits growth of organisms introduced when the closure is pierced. That single difference is what turns a single-entry container into one whose labelling contemplates repeated entry, and it is why the two are not interchangeable despite looking identical.
Neither is isotonic, which matters more than the preservative for some uses. Plain water is hypotonic and haemolytic given directly into a vein, so both articles are labelled for use only after admixture. The benzyl alcohol carries its own restriction: it is contraindicated in neonates after the gasping syndrome deaths reported in the early 1980s.
The distinction decides what a container may become. A preservative-free diluent yields a preparation with no antimicrobial defence, so its in-use window is governed by microbial ingress rather than chemical stability, and compounding standards treat it accordingly. A preserved diluent widens that window but adds an excipient that can interact with the drug, and some proteins aggregate in the presence of benzyl alcohol.
The recurring grey-market error is treating bacteriostatic water as a sterilising agent or a universal solvent that makes any preparation keepable. It is bacteriostatic, not bactericidal and not antifungal, and it does nothing for endotoxin, which is heat-stable and unaffected by preservatives. A second confusion is with bacteriostatic sodium chloride, a different article containing salt and often a different preservative, whose compatibility profile differs.
