Excipient

An excipient is any component of a formulation other than the active ingredient, including buffers, tonicity agents, surfactants, preservatives and bulking agents, and none of them are truly inert.

Excipients are the components that turn a drug into a product. An injectable peptide formulation typically carries a buffer to fix pH, a tonicity agent so the solution is not haemolytic, a surfactant to protect against interfacial damage, a preservative if the container is multi-dose, and in a lyophilised product a bulking agent and lyoprotectant that structure the cake and keep the molecule folded. Regulators publish an inactive ingredient database because prior use at a given level by a given route is part of how new formulations are justified.

Inactive is a legal category, not a pharmacological claim. Citrate buffers are associated with injection pain, which drove citrate-free reformulations of several subcutaneous biologics marketed on comfort grounds. Polysorbate 20 and 80 degrade by oxidation and hydrolysis into fatty acids that can themselves form particles. Polyethylene glycol and polysorbates have both been implicated in hypersensitivity reactions.

Reading an excipient list therefore tells you how a product is meant to behave. A preservative signals a multi-dose presentation; a surfactant signals a molecule vulnerable to shaking and interfaces; mannitol or sucrose signals a lyophilised cake; a low pH may signal a solubility strategy or a precipitating depot. Differences of this kind are also why two products containing the same peptide are not automatically substitutable.

The error to name is dismissing an adverse reaction as impossible because the excipients are inactive, or blaming every reaction on them. Both are lazy. The more specific peptide mistake is reading an excipient list on a certificate of analysis as evidence of a finished formulation: research-grade material commonly carries residual synthesis components such as trifluoroacetate counterion, process residues with no compatibility data behind them.
