Tonicity Agent

A tonicity agent is an excipient added to bring a parenteral solution close to the osmotic pressure of body fluids, so cells at the injection site neither swell nor shrink.

A tonicity agent is an excipient added solely to adjust osmotic pressure. Plasma sits near 285 to 295 milliosmoles per kilogram, and a solution injected into tissue draws water out of or into local cells depending on which side of that figure it falls. Osmotic pressure is colligative, depending on the number of dissolved particles rather than their identity, which is why sodium chloride contributes roughly twice per formula unit and a sugar once. The usual agents are sodium chloride, mannitol, glycine, sucrose and trehalose, and in lyophilised products the sugars double as lyoprotectants.

Tonicity and osmolarity are not interchangeable, and the distinction is chemically real. Tonicity counts only solutes that cannot cross the cell membrane. A urea solution can be iso-osmotic with plasma yet functionally hypotonic, because urea equilibrates across the membrane and water follows the remaining gradient. Formulation records therefore state the measured osmolality and the intended tonicity separately.

The choice of agent is rarely neutral for the drug. Ionic strength from sodium chloride screens the electrostatic repulsion that keeps a peptide monomeric, promoting aggregation in some molecules and suppressing it in others, while sugars generally stabilise but raise viscosity. Tonicity also contributes to injection-site tolerability alongside pH and buffer choice.

The misstep seen in unregulated preparation is assuming that dissolving a powder in plain water yields a physiologically appropriate solution. It yields whatever tonicity the solid alone provides, which for a small mass of peptide is close to none, and hypotonic injections are more irritating than isotonic ones. Equally common is treating isotonic as a synonym for physiological, when pH, buffer species and preservative content are separate questions.
