Wrinkle Depth Measurement (Profilometry)

Profilometry quantifies skin surface topography, from a silicone replica or by direct optical scanning, yielding roughness parameters used as objective wrinkle endpoints in cosmetic trials.

There are two families of method. The indirect approach casts a negative replica of a defined site, classically the crow's feet area, in a fast-setting silicone elastomer, then measures the cast by stylus, laser or optical scanning off the body. The direct approach images living skin in situ, most often by fringe projection, in which a striped pattern is thrown onto the surface and its distortion reconstructed into a three-dimensional height map. Both yield a surface profile from which roughness parameters are computed.

Those parameters come from engineering surface metrology rather than dermatology. The arithmetic mean deviation of the profile and the mean peak-to-valley height are the usual reported measures, sometimes with derived wrinkle depth, area and volume for a segmented furrow. Cosmetic studies typically report changes from a few percent up to roughly a fifth of baseline roughness over eight to twelve weeks, uncomfortably close to the measurement error of the technique.

Profilometry is more sensitive and more reproducible than an investigator's ordinal wrinkle grade, but sensitivity is not meaning. A statistically significant reduction in a roughness parameter that no blinded observer and no participant can see is a measurement, not a benefit, which is why serious programmes pair topography with a validated clinical scale and a subject-reported outcome and ask what magnitude would be perceptible.

The dominant artefact is hydration. Water uptake swells the stratum corneum and flattens fine surface lines within an hour, so a measurement taken after applying a product, or without standardised washout and acclimatisation, cannot separate moisturisation from structural remodelling. Replica methods add their own: skin tension at casting, mould pressure, and any difference in facial position between visits alter the profile before treatment does.
