Thymic Peptide

Thymic peptides are small peptides originally isolated from thymus tissue and proposed as immune-regulating hormones; only thymosin alpha-1 has meaningful clinical use, and it is not FDA-approved.

Thymic peptide is a historical category rather than a chemical one. In the 1960s and 1970s crude thymus extracts were fractionated in a search for the hormone behind T-cell education, yielding thymosin alpha-1, thymosin beta-4, thymulin and thymopentin. Only some proved thymus-specific or hormone-like. Thymosin alpha-1 is a 28-residue peptide with an acetylated N-terminus and real immunomodulatory activity on dendritic and T cells. Thymosin beta-4 came from the same preparations but is an actin-sequestering protein present in almost every cell, with no hormone function.

Their regulatory positions differ sharply. Thymosin alpha-1 is licensed in a number of countries as an adjunct in chronic hepatitis B and has been studied in sepsis, but has never been approved by the FDA, and in 2023 it appeared among substances the agency placed in the compounding category for significant safety concerns. Thymosin beta-4 holds no approval anywhere and circulates as a research chemical, usually as a partial sequence sold under a trade code.

The category label is where the trouble starts. Grouping a clinically used immunomodulator with an unrelated cytoskeletal protein transfers credibility only one of them earned, and both are then offered for indications neither has been tested in. Approval in one jurisdiction is not approval generally: a compound licensed abroad remains an unapproved new drug elsewhere.

The specific error is assuming that a vial labelled with the thymosin beta-4 trade code contains thymosin beta-4. The circulating product is a fragment of the parent sequence, while the animal data most often cited used the full protein. The category rests on an appealing premise, that supplying a thymic factor restores an ageing immune system, for which no controlled human outcome trial exists.
