Immunomodulation
Immunomodulation is the deliberate shifting of immune activity in either direction, a category covering suppression, stimulation and restored regulation, often used precisely to avoid naming which is meant.
Immunomodulation is an umbrella rather than a mechanism. It covers cytokine agonists and antagonists, pattern-recognition receptor agonists, checkpoint modulation and regulatory cell expansion. It is distinguished from immunosuppression, which has one direction, and from immunostimulation, which has the other. Genuine bidirectionality exists for a minority of agents, usually because they act on a regulatory node rather than on effector output.
Approved examples span the range. Imiquimod, a TLR7 agonist, is applied topically for actinic keratosis and superficial basal cell carcinoma; BCG instilled into the bladder treats non-muscle-invasive bladder cancer; interferon beta is used in multiple sclerosis. Peptide examples are thinner than the marketing suggests. Thymosin alpha-1 is licensed in several countries for chronic hepatitis B and as a vaccine adjuvant but has never been FDA-approved, and its trials in sepsis and severe viral illness have been small and inconsistent.
The question worth asking of any immunomodulatory claim is which arm, in which direction, measured how, and in which species. An agent that raises NK cytotoxicity in a cell-killing assay and one that expands regulatory T cells are both accurately called immunomodulatory while having opposite clinical consequences. Direction also sets the risk profile: stimulation risks autoimmunity and cytokine release, suppression risks infection and reduced tumour surveillance.
In supplement and grey-market copy the word functions as an unfalsifiable claim, because any change in any immune parameter can be presented as confirmation. It also does regulatory work, since a structure-function claim about supporting immune balance sits inside dietary supplement rules while a claim to treat an immune disease does not, so the vaguer term is chosen deliberately.
