Slow-Wave Sleep (SWS)

Slow-wave sleep is stage N3, the deepest non-REM sleep, scored when high-amplitude delta activity fills at least a fifth of an epoch and associated with growth hormone release and memory consolidation.

Slow-wave sleep is scored as stage N3 when high-amplitude, low-frequency delta activity occupies at least twenty percent of a thirty-second epoch. It reflects synchronised alternation of cortical neurons between depolarised up-states and hyperpolarised down-states, which is why the electroencephalogram looks so unlike waking. Arousal from it is hardest and confusion on waking greatest, and it is homeostatically controlled: the longer prior wakefulness, the more of it appears, and it rebounds after deprivation. It is concentrated in the first cycles of the night and declines markedly across the adult lifespan.

Two functional associations make it clinically interesting. Most of the day's growth hormone is released in pulses tied to the first slow-wave episode, which is why the somatotropic axis is discussed alongside sleep. Separately, slow oscillations and sleep spindles are thought to coordinate the transfer of newly encoded hippocampal memories to cortex, the mechanism behind sleep-dependent consolidation. Pharmacologically, sodium oxybate reliably increases slow-wave sleep and is approved for narcolepsy with cataplexy, though it is a controlled substance under a restricted programme.

The useful caution is that more slow-wave sleep is not automatically better. It has a homeostatic set point, and drugs that force it can leave people unrefreshed, while the growth hormone pulse it carries is a physiological event, not a therapeutic outcome.

That last point is where growth hormone secretagogue and GHRH-analogue marketing goes wrong. An observed increase in slow-wave sleep is presented as proof of deeper rest, then the associated growth hormone pulse is presented as evidence of anti-ageing benefit, without any clinical endpoint at either step. Wearable deep-sleep minutes are not N3 and cannot support the first claim either.
