GHRH Analogue
A GHRH analogue is a synthetic peptide that stimulates pituitary somatotrophs at the GHRH receptor, usually built as a stabilised fragment of the native 44-residue hypothalamic hormone.
Growth hormone releasing hormone is a 44-amino-acid hypothalamic peptide whose full biological activity resides in the first 29 residues, which is why almost every analogue is built on the 1-29 fragment. The native peptide is cleaved within minutes at the alanine in position 2 by dipeptidyl peptidase-4, so analogue design is largely about protecting that bond. Acting at the GHRH receptor on the somatotroph, these compounds raise cyclic AMP and promote both synthesis and release of growth hormone.
Two matter clinically. Sermorelin, which is GHRH(1-29), was approved in the United States in the 1990s and withdrawn from the market in 2008 for commercial rather than safety reasons. Tesamorelin, a derivative of the full 44-residue sequence, was approved by the FDA in 2010 for excess visceral abdominal fat in HIV-associated lipodystrophy, and is the only GHRH analogue approved as a therapy there. CJC-1295 comes from a different lineage: the version bearing a drug affinity complex carries a maleimide group that binds covalently to circulating albumin, extending its half-life from minutes to days.
That half-life extension is where the pharmacological argument sits. A short-acting analogue superimposes a pulse on an axis that signals in pulses, whereas an albumin-bound construct produces continuous receptor occupancy. Pulsatile and tonic exposure are not interchangeable here, and no controlled outcome data exist for the tonic versions.
The claim to watch is that sermorelin is FDA-approved. The approval existed, for a product not marketed since 2008, and material compounded under that name today is not an approved drug. Separately, the widely repeated synergy between a GHRH analogue and a releasing peptide is mechanistically plausible, since they act at different receptors, but a larger hormone pulse is not itself evidence of clinical benefit.
