Lean Mass Loss

Lean mass loss is the fat-free component of weight lost during an energy deficit, encompassing muscle, organ tissue, glycogen and water rather than muscle alone.

Lean mass, or fat-free mass, is everything the body is not storing as fat: skeletal muscle, organs, bone mineral, connective tissue and the water bound up with all of it. Weight loss is never purely fat. In sustained energy deficit the fat-free share of what is lost typically falls somewhere between a fifth and two fifths, varying with the size of the deficit, protein intake, resistance training and starting body composition. Dual-energy X-ray absorptiometry reports lean soft tissue, which is not a muscle measurement.

The body composition substudy of the 68-week STEP 1 semaglutide trial is the usual reference point: total fat mass fell, lean mass fell as well, and yet the proportion of the body that was lean rose, because fat made up the larger share of the loss. Bariatric surgery and plain caloric restriction produce broadly similar ratios. Interest in add-on agents that spare or add lean tissue, including myostatin and activin pathway antibodies tested alongside incretins, follows directly from those numbers.

The question worth asking is not whether lean mass fell but whether it fell more than the amount of weight lost would predict, and whether function changed with it. Grip strength, gait speed and stair-climb power are the endpoints that carry meaning; a kilogram of fat-free mass shed alongside twelve kilograms of body weight in someone whose strength is unchanged is not the same event as the same loss in a frail seventy-five-year-old.

Two things inflate apparent lean loss. Glycogen is stored with roughly three parts water, so the first weeks of any deficit drop several kilograms of fat-free mass that is largely water and returns on refeeding. And absorptiometry hydration assumptions drift during rapid weight change. Meanwhile, products advertised as muscle-sparing rarely present any composition endpoint at all, let alone a functional one.
