Myostatin

Myostatin is a muscle-secreted member of the TGF-beta family that acts as a brake on muscle growth, and blocking it enlarges muscle far more reliably than it improves function.

Myostatin, also called growth differentiation factor 8, is a TGF-beta superfamily protein secreted by skeletal muscle that restrains its own growth. It circulates as a latent complex with its propeptide, and once freed it binds activin type II receptors and signals through SMAD2 and SMAD3. It suppresses the Akt and mTOR pathway that drives protein synthesis and raises ubiquitin-ligase-driven degradation, so it lowers muscle mass from both directions.

The loss-of-function phenotypes are dramatic. Belgian Blue and Piedmontese cattle carry myostatin mutations and show the double-muscled conformation, as do certain whippets, and a single human case of a child with a myostatin mutation and markedly increased muscle mass has been reported. The therapeutic record is the opposite of dramatic. Antibodies and receptor traps aimed at this axis have been tested in muscular dystrophy, inclusion body myositis, sarcopenia and cancer cachexia; they reliably added lean mass on imaging and repeatedly failed to improve walking distance, strength or other functional endpoints, with several programmes discontinued after phase 2 or phase 3.

That pattern is the whole lesson. Enlarging a muscle by removing a brake does not necessarily give it more usable force, because tendon, innervation, fibre quality and motor control are unchanged. A drug class with clean target engagement and a visible imaging effect still failed the endpoint patients care about.

The grey market has not absorbed this. Compounds sold as myostatin inhibitors, follistatin-derived products among them, rest on animal work and the cattle phenotype, not on any human trial, and serum myostatin assays are unreliable because they struggle to separate latent from active protein. A reported drop in circulating myostatin is not evidence of anything a person will notice.
