Intramuscular Injection (IM)

Intramuscular injection places drug into skeletal muscle, a more vascular and higher-capacity compartment than subcutaneous tissue that generally yields faster absorption of aqueous solutions.

An intramuscular injection deposits drug beneath the subcutaneous fat, into a skeletal muscle such as the deltoid, the vastus lateralis or the gluteal muscles. Muscle has a denser capillary network and higher resting blood flow per unit mass than adipose tissue, so an aqueous solution placed there is typically absorbed faster and peaks sooner than one placed subcutaneously. Muscle also accommodates larger volumes with less pressure and pain. The compartment is not constant: local blood flow rises with exercise and heat and falls in shock.

The route is also the home of oil-based and suspension depots, where the reservoir rather than the tissue governs rate. Testosterone esters in oil and long-acting antipsychotics are given intramuscularly for that reason, as are leuprolide microsphere formulations and most vaccines, where the antigen-presenting environment of muscle is part of the rationale. For peptides in aqueous solution the route is less common than the subcutaneous one.

Whether the needle actually reaches muscle decides everything else. Imaging studies show subcutaneous fat thickness varying several-fold between individuals, so a needle sized for an average patient can land in fat in one person and muscle in another. An injection intended as intramuscular that lands in fat behaves like a subcutaneous one, altering absorption and, for irritant or depot formulations, raising the risk of a local reaction.

The error that matters most is assuming intramuscular is inherently the stronger route. It changes the shape of the concentration curve, not the total amount absorbed, and for a depot it can be slower rather than faster. The route also carries hazards seldom discussed outside clinical training, including nerve injury at poorly chosen sites, inadvertent intravascular delivery of an oil-based product, and sterile abscess with irritant vehicles.
