Injection Site Rotation

Injection site rotation is the practice of distributing subcutaneous injections across tissue rather than repeating them in one spot, to prevent local tissue change that makes absorption erratic.

Repeated injection into the same small area of subcutaneous tissue changes the tissue itself. The best characterised change is lipohypertrophy, a rubbery thickening of subcutaneous fat driven partly by the local anabolic effect of insulin and partly by mechanical trauma. Because the altered tissue is poorly vascularised, drug deposited into it is absorbed more slowly and far less predictably than drug placed in normal tissue. Rotation is simply the principle of not loading one tissue bed repeatedly.

In insulin therapy the effect is well quantified. Clinic surveys using structured palpation have found lipohypertrophy in a substantial fraction of long-term injectors, and comparative studies show absorption from affected tissue is both reduced and considerably more variable between occasions. That variability shows up as unexplained glycaemic swings, and education programmes addressing site use and needle reuse have reported falls in insulin requirement.

This matters beyond comfort because it breaks the assumption underlying every dose decision, that the same administration produces roughly the same exposure. Once a proportion of administrations lands in altered tissue, exposure is not merely lower but inconsistent, and the resulting instability is easily misread as the drug losing effect or as a change in the underlying condition.

The specific misconception is that rotating within one small area counts as rotation. Moving a few millimetres inside a favoured patch still deposits into the same tissue bed, while absorption differs measurably between abdomen, thigh and arm, so drifting between regions unsystematically introduces variability of its own. Needle reuse, which blunts and deforms the tip, feeds the same local trauma and is usually discussed as though it were only a sterility question.
