Nootropic

A nootropic is any substance claimed to improve cognition, a term coined for a specific pharmacological profile that has since become a marketing category with no regulatory meaning.

The word was coined by Corneliu Giurgea in the early 1970s, around piracetam, and it carried a definition: enhance learning and memory, protect the brain against injury, facilitate cortical control mechanisms, show very low toxicity, and do all of this without the sedation or stimulation of conventional psychoactive drugs. Almost nothing now sold as a nootropic satisfies that last criterion, and the term has drifted to mean any compound marketed for cognition, from caffeine to prescription stimulants to unapproved research peptides.

Regulatory status inside the category is heterogeneous and worth separating. Donepezil and rivastigmine are approved drugs for Alzheimer's disease. Modafinil is an approved prescription drug for narcolepsy, obstructive sleep apnoea and shift work disorder, and is a controlled substance; it is not approved for cognitive enhancement in healthy people. Piracetam is not an approved drug in the United States and the FDA has said it does not qualify as a dietary ingredient. Various peptides in this space are unapproved and sold labelled for research use only.

The question that separates a real claim from a hollow one is which population and which endpoint. Agents that improve cognition in a deficient state, whether from sleep deprivation, cholinergic loss or hypoxia, often do nothing in rested healthy adults, who are already near their ceiling on most tasks. Effects also tend to be domain-specific: vigilance and reaction time move more readily than working memory or episodic recall.

The misreading to watch is subjective alertness mistaken for measured cognition. A user who feels sharper reports a nootropic effect, but arousal, mood and effort all inflate self-report while leaving objective test scores unchanged, and unblinded self-experimentation cannot distinguish the two.
