Neurotrophic Factor

Neurotrophic factors are secreted proteins that support the survival, growth and maintenance of neurons, signalling through Trk and related receptors to keep target-connected cells alive.

Neurotrophic factors are secreted proteins that neurons depend on for survival and maintenance. The neurotrophin family proper comprises nerve growth factor, brain-derived neurotrophic factor, neurotrophin-3 and neurotrophin-4, signalling through the tyrosine kinase receptors TrkA, TrkB and TrkC and through p75, which can instead promote cell death. Related but structurally distinct families include glial cell line-derived neurotrophic factor and ciliary neurotrophic factor. The founding idea, from Levi-Montalcini and Cohen's Nobel-recognised work, is that developing neurons compete for a limited supply of target-derived factor and those that fail to obtain it die.

Translating that into therapy has been difficult. Recombinant ciliary neurotrophic factor and recombinant brain-derived neurotrophic factor both failed in amyotrophic lateral sclerosis trials, the latter also causing injection-site effects. Direct intraputaminal infusion of glial cell line-derived neurotrophic factor in Parkinson's disease missed its primary endpoint despite a plausible imaging signal. The one clear approval is cenegermin, a recombinant human nerve growth factor given as an eye drop for neurotrophic keratitis, which acts on the cornea and never has to reach the brain.

That pattern points at the real constraint. These are proteins of tens of kilodaltons; they do not cross the blood-brain barrier in meaningful quantity, are cleared quickly, and produce pain and weight loss when given systemically at active doses. Any neurotrophic claim therefore owes an account of how the factor, or a signal raising it, reaches the target tissue.

The frequent overreach is a compound called neurotrophic because it raised a serum neurotrophin value or increased neurite outgrowth in a dish, neither of which establishes neuronal survival in a living brain.
