Enteroendocrine L-Cell

Enteroendocrine L-cells are hormone-secreting intestinal epithelial cells that release GLP-1, GLP-2, oxyntomodulin and peptide YY in response to luminal nutrients.

L-cells are open-type enteroendocrine cells in the intestinal epithelium, with an apical face exposed to the lumen and secretory granules at the basolateral pole. They are densest in the distal ileum and colon and detect nutrients directly through sugar transporters, free fatty acid and bile acid receptors and amino acid sensors rather than a neural relay. Their principal products come from proglucagon, which the L-cell processes with prohormone convertase 1/3 into GLP-1, GLP-2 and oxyntomodulin; peptide YY is co-released.

The same proglucagon gene in the pancreatic alpha cell is processed by prohormone convertase 2 into glucagon instead, the cleanest example of one transcript giving opposite hormones by cell-specific processing. L-cells make up well under one percent of gut epithelium, and the related K-cells of the duodenum and jejunum handle GIP. Roux-en-Y gastric bypass delivers undigested nutrients directly to the distal bowel and raises postprandial GLP-1 several-fold, one of the most reproducible findings in bariatric physiology.

Anatomy explains why the incretin response depends on a nutrient's route. Oral glucose recruits L-cells and K-cells; an intravenous infusion matched for plasma glucose does not, and the insulin difference between the two is the incretin effect itself. It also explains why drugging the L-cell directly, through bile acid or fatty acid receptor agonists, has produced smaller effects than agonism downstream.

The commercial misreading is the promise of boosting your own GLP-1 naturally. Even a several-fold rise in endogenous secretion yields concentrations far below those of a weekly agonist, lasts minutes rather than days, and is largely destroyed by DPP-4 before reaching the systemic circulation. A supplement that measurably increases L-cell secretion has still shown nothing pharmacological.
