Cytokine Storm

Cytokine storm is a hyperinflammatory syndrome in which unrestrained cytokine production, typically involving interleukin-6 and interferon gamma, causes fever, hypotension and organ dysfunction.

Cytokine storm is a self-amplifying loop between activated T or NK cells and myeloid cells in which normal negative regulation of cytokine production fails. Interleukin-6, interferon gamma, tumour necrosis factor and interleukin-1 are the usual leading actors, and endothelial activation converts that signalling into capillary leak, hypotension and coagulopathy. It is defined clinically, by fever, hypoxia, vasopressor requirement and organ dysfunction, not by any threshold cytokine concentration.

Chimeric antigen receptor T-cell therapy made the syndrome routine: most treated patients develop some grade of cytokine release syndrome, graded one to four by the ASTCT consensus criteria on fever, oxygen and vasopressor requirements, and tocilizumab was approved in 2017 specifically for it. The starkest example remains TGN1412, a CD28 superagonist antibody, which in a 2006 first-in-human study put all six dosed volunteers into multi-organ failure within hours at a dose hundreds of times below the level found safe in macaques.

That episode changed trial design permanently. First-in-human dosing for immunostimulatory biologics moved from scaling down an animal no-effect dose to calculating a minimum anticipated biological effect level, and sequential rather than simultaneous dosing of volunteers became standard.

The term is now applied to any panel with high cytokines, which drains it of meaning. A raised interleukin-6 in a septic or post-operative patient is the expected acute-phase response, not a storm; the label requires the systemic syndrome plus evidence the cytokines are driving the pathology. In the peptide world the error runs the opposite way, with immune-stimulating compounds assumed to be gentle because they are peptides, when that is the class which has produced the worst human cytokine storms.
