Cold Chain

The cold chain is not a refrigerator; it is an unbroken, documented system spanning manufacture, warehousing, transport and the point of use, within which a product stays inside the temperature range its label specifies. For most peptide and protein medicines that range is two to eight degrees Celsius, with some requiring frozen storage and a defined in-use period at ambient once opened. A departure is a temperature excursion, defined by magnitude and duration together, since both determine how much degradation occurred.

The engineering is unremarkable and the documentation is the point. Validated shippers are qualified against seasonal temperature profiles, phase-change materials are conditioned to specification, and electronic loggers travel with the shipment so the whole transit history can be reviewed. The approved incretin analogue pens carry this pattern on their labelling: refrigerated storage before first use, then a bounded number of days at room temperature, the second figure drawn from real stability data rather than convention.

Refrigeration does not preserve a peptide; it slows chemical processes whose rates rise with temperature. Whether a particular excursion matters is a question only that product's stability data can answer. This is why excursion assessment is a documented, product-specific judgement, and why a product with no stability programme cannot have its excursions assessed at all.

The mistake is judging an excursion by inspection. Deamidation, oxidation and low-level aggregation leave a solution looking unchanged, and a clear, particle-free vial can have lost potency. In the grey market the standard defence of ambient shipping is that lyophilised powder is stable, broadly true for many peptides over short periods and still a generic assertion standing in for lot-specific data nobody generated.
